Patients diagnosed by a healthcare provider (Doctor of Medicine and or APRN in good standing) qualified by the State of Kentucky Cannabis Program diagnosed the individual has been with a qualifying identified medical condition including:

Any type or form of cancer; chronic or severe pain; epilepsy or other intractable seizure disorder; multiple sclerosis, muscle spasms, or spasticity; chronic nausea or cyclical vomiting syndrome, post-traumatic stress disorder, or any terminal illness.

With the doctor’s approval, you’ll receive your Kentucky medical marijuana certification.

If You Believe you’re eligible for a Medical Marijuana Cannabis card in Kentucky the next step is to obtain a cannabis certification. One of our physicians/APRN’s will perform an examination and determine if you have a qualifying Medical condition. 

At this time the physician/APRN will indicate the product type and quantity of your medical marijuana you’ll be authorized to obtain. You’ll need to provide your email address to the doctor/APRN so that you can create your New Patient account, upload your notarized signature page, and receive updates from the registry.
